Le dosage sérique de la protéine S100β comme marqueur pronostique après rupture de Malformation artérioveineuse cérébrale

Abstract : Background : Clinical, demographic and imaging prognostic markers after brain arteriovenous malformation (BAVM) rupture have been repeatedly reported. Biological markers, on the other hand, are lacking. The purpose of this study is to determine whether elevation of S100β protein serum level is associated with increased inhospital mortality after BAVM rupture. Methods : This is a retrospective monocentric study of patients admitted for BAVM rupture in a tertiary-care center between 2003 and 2018. Demographic, clinical, imaging and biological data was analyzed. Association between S100β protein serum level and inhospital mortality was studied. The ethics committee of our institution approved this study. The need for patients’ informed consent for retrospective analyses of records and imaging data was waived. Results : One hundred ninety-two BAVM rupture events met inclusion criteria. Twenty-one hemorrhages led to inhospital mortality (11%). The mean highest S100β serum concentration within the first 48 hours after admission (S100βmax48) was 0.5±0.63 µg/l. multivariate stepwise logistic regression found only initial Glasgow coma scale≤8 (P=0.011, OR 10, 95%CI [3-42]) and an S100Bmax48>0.5µg/l (P=0.008, OR 7, 95%CI [2-28]) to be independently and significantly associated with inhospital mortality. Conclusions: In our cohort, an elevated S100B protein serum level is strongly predictive of inhospital mortality after BAVM rupture. A validation cohort study should be necessary to expand our results to the clinical practice.
