Thérapie cognitivo-comportementale du souci : une approche trans diagnostique possible ?

Abstract : Worry is a commonplace and ordinary repetitive thinking process. Depending on its intensity, it can be percieved as uncontrollable, interfere with one’s fonctionning, and so become pathological. It’s the case in the generalized anxiety disorder of whom it is the central symptom. At a time when, a dimensional view of mental disorders begin to emerge, worry as a repetitive negative thought, such as rumination, has been apprehended as a transdiagnostic process, found in several axis I disorders, and associated with the presence of negative affects. After a comparative presentation of different repetitive thoughts and a description of the main explicative models of worry, the aim of this review of litterature is to study the extent to which such an approach can be valid in cognitive and behavioral therapy, compared to the usual categorial view. Despite promissing studies, proofs concerning the efficacity of these complaint and process based therapy are insufficient, because of the few articles on the subject, as the main research concerns the generalized anxiety disorder. Furthermore, other approaches than cognitive and behavioural therapy can be interesting to target this process, such as mindfulness.
