Le risque cardiovasculaire chez le personnel navigant : corrélation entre estimation du médecin aéronautique et scores de référence

Abstract : Cardiovascular risk assessment is an every day concern for the flight surgeon considering the disastrous consequences of a cardiovascular incident on flight safety. To facilitate the identification of higher cardiovascular risk profile, using a cardiovascular risk assessment score is recommended. However, empirical evaluation is often preferred. The goals of this study are to evaluate the correlation between flight surgeon’s estimation and score’s then analyse the causes of misestimated populations. The cardiovascular risk estimation was collected for every over 40 aircrew members that get an aeronautical fitness evaluation between 04.01.2017 and 03.31.2018 in Bordeaux’s aeromedical center. 564 aircrew members was included, comprising 372(66%) military men, 149(26,4%) male civilians, 21(3,7%) military women and 22(3,9%) female civilians (46,6±6,2 y.o. on average). Flight surgeon’s estimations was strongly correlated to score’s estimations (ESC-SCORE, Framngham 2008, Framingham without laboratory tests and ASCVD). The correlation was better when the goal is the identification of low risk patients. Flight surgeons tend to underestimate the age as a cardiovascular risk factor and the cumulative risk of moderate to elevate biometrics datas. Each method complete the other one, every patient must be evaluated separately with caution, prioritizing cardiovascular disease screening for flight safety.
