Enquête sur l’activité dermatologique du médecin généraliste en Aquitaine et la nécessite d’un avis dermatologique ou télédermatologique

Abstract : Introduction: the need for a dermatological consultation presents us with problems of distance and access to a dermatologist and the possibility of some waiting. Teledermatology could be a worthwhile and accessible tool for the general practioner (GP) in terms of diagnosis and therapy. The aim of our study was to define the GPs' needs for teledermatology in Aquitaine before setting up such a system. Methods: we have carried out a prospective practice investigation through an online questionnaire bearing on the dermatological activity of the Academic Internship Supervisor General Practitioner (AISGP) in Aquitaine. In the course of one week in November and one week in December, each training doctor in level 1 internship filled in an online questionnaire on the reasons for dermatological consultation and the need for a dermatological or teledermatological informed opinion by the GP observed Results: in all, 141 pairs of training doctor/AISGP have answered the questionnaire. Out of 6533 consultations, 434 involved a dermatological complaint, which corresponds to an occurrence rate of 6.6 %. In 77.9% of the cases, the GP was able to give a diagnosis with an average degree of certainty of 8.6 out of 10. The majority of the causes were infectious (27.6%), allergic (15.7%) and traumatic (9.7%). No second opinion was needed in 65.2% of the 434 cases. When it was found necessary, it was a teledermatological opinion in 28.3% of the cases and a dermatological one in 14.7%. The AISGP's age and experience do not seem to have had an influence on the need for a teledermatological second opinion. The Gps preferred this technique to the dermatological consultation in the case of pathologies assumed to be infectious or allergic, but showed no preference in the case of tumoral pathologies. Conclusion: teledermatology can be an alternative to dermatological consultation with a restriction in the cases of tumoral pathologies. Experimentation is needed to assess the feasibility of this new method.
