Évolution de la douleur et des indices fonctionnels lors du traitement des tendinopathies par ondes de choc : étude en médecine générale

Abstract : Background: tendinopathies are a reason for frequent consultation in general practice. To date, there are no specific recommendations on how to take care of them. Shockwaves are one of the possible treatments for these pathologies, but they have been little evaluated. OBJECTIVE: to determine the progression of pain, functional scores and quality of life after 4 to 6 shock wave sessions in patients with tendinopathy. Method: longitudinal descriptive study followed by a cohort of patients treated by shock wave for different tendinopathies (lateral elbow epicondylitis, plantar fasciitis, rotator cuff tendinitis and Achilles tendinopathy). Results: 129 patients were included. Six weeks after the end of the treatment, the EVA score is statistically significantly improved for the four locations. It ranges from an average of 7.8 to 0.4 for epicondylitis, 8.4 versus 0.3 for plantar fasciitis, 7.8 versus 0.9 for rotator cuff tendinitis and 7,5 against 0.9 for Achilles tendinopathy. There is also significant clinical improvement regardless of the localization of tendinopathy. On the other hand, only the physical component of quality of life is statistically significantly improved and not the social and mental components. Conclusion: extracorporeal shock wave therapy is associated with a significant decrease in pain and significant clinical improvement. It is therefore a non-invasive treatment that could be offered as first-line primary care. Its ease of realization makes it usable by the general practitioners.
