Douleurs cancéreuses intenses en situations palliatives symptomatique et spécifique à domicile : rôles et fonctions des proches aidants

Abstract : Context: the number of people living with a cancer at home is increasing. Caregivers are essentials to the patient in this context. To this end, they are exposed to severe cancer pain. Pain is the most frequent symptom in oncology patients, frequently not enough relieved. The aim of this study is to know better the role and the functions of caregivers at home, especially regarding cancer pain and pain management. Methods: a qualitative study and semi-structured face-to-face was conducted with caregivers facing severe cancer pain with patients in palliative care who needed opioids at home. Themes approached were : communication, anticipation, organization, support and adaptation. Results: 12 caregivers has been interviewed. Most caregivers state that they try to be the most available for the wellbeing of the patient, monitoring of symptoms and use of opioids. These are sources of a lot of issues. Furthermore, caregivers are feeling responsible for striving and organizing to face the pain. Finally, they feel to be the best person to give an everyday support to the patient, to a practical and an emotional level. Care professionals, especially nurses, are considered to be important resources on which they can rely. Discussion: the issue for caregivers is to justify their role in cancer pain management at home. Communication, organization and decision making are part of their coping strategies but they are variables in time. The key is to find the balance in the burden which weighs on them. Health care professionals providing home care should pay close attention to their needs by offering them an appropriate and flexible help.
