Évaluation des compétences des médecins généralistes dans la lecture et l’interprétation d’un électrocardiogramme 12 dérivations

Abstract : Introduction: the electrocardiogram has many indications in general practice. The aim of our study is to assess the ability of general practitioners in France to recognize electrocardiographic abnormalities. Methods: we conducted a descriptive, quantitative study, using a questionnaire, open to all physicians registered as generalists and practicing in France in 2018. General information was requested and then 16 ECG were presented. Results: 227 physicians in 51 departments responded. Physicians' activity patterns were divided into three groups: 77 (34%) physicians practiced exclusively in private practice. 95 (42%) made home visits. 55 (24%) had a hospital activity. There is no difference in level when comparing the 3 groups with each other (p = 0.097). 67.6% of the diagnoses were found. The number of ECGs performed per year and the estimated level are correlated with the scores (p = 0.02 and p <0.001). Conduction disorders have often been confused with each other. 83% of physicians did not recognize atrial extrasystoles. 84% of the doctors found the serious diseases and 93.4% oriented them correctly. Discussion: physicians got good answers, especially on serious illnesses. They were able to refer patients when necessary. There are, however, gaps in conduction disorders and atrial extrasystoles. Conclusion: GPs should continue to perform ECGs because their skills are good and the indications numerous. The sharing of ECG between doctors is a good way to maintain the knowledge.
