La télémédecine en psychiatrie : pratiques et représentations chez les psychiatres normands

Abstract : Introduction: Telemedicine is defined as the practice of medicine at a distance through the use of information and communication technologies (ICT). It includes teleconsultation, teleexpertise, remote assistance, remote monitoring and medical regulation. Faced with increasing inequalities in healthcare and a worrying medical demography, the public authorities are in favour of deploying these acts. But the practice of telemedicine in psychiatry is not without its questions and controversies. Materials and methods: Our study consisted in evaluating the practices and representations of the 531 Norman psychiatrists and child psychiatrists registered with the Order on January 1, 2019 via a questionnaire sent by e-mail or post. Results: 181 psychiatrists responded to the questionnaire. 55.8% of respondents said that teleconsultation was possible and 47.2% said they were willing to do it themselves. The use of teleconferencing was perceived very favourably with 35.6% having already done so and 67.3% who would like to do so. On the other hand, remote expertise, with 18.9% intention to practice, is not very much considered by Norman psychiatrists. Geographical distance is a factor influencing practices and representations. The main opponents of teleconsultation deplore the loss of human contact. Conclusion: Although the teletaff is very well received by Norman psychiatrists, teleconsultation is still perceived in a mixed way, for fear of loss of clinical information or lack of ease with the tool in particular. Positive feedback from ongoing experiences or the delivery of telemedicine courses to the faculty could help to overcome some of the concerns.
