Le rôle de la thèse d'exercice dans le D.E.S. de médecine générale en France depuis 2004, pour ses différents acteurs : revue de la littérature et analyse de contenu

Abstract : Introduction: MD thesis are a French particularity that medical interns defend at the end of their formal education. General practice works have been made about its stakeholders’ experience and its role. We have formulated the hypothesis that, examined equally, all these works enable to first, understand the role assigned to the MD thesis in France, and secondarily, to list the difference of opinion between the stakeholders about this role. Aim. Exploring the role assigned to the MD thesis by its stakeholders since the creation of the post-graduate specialist diploma (entitled D.E.S.) in 2004, in France. Methods: a review of the literature has been made, then a category thematic content analysis. Results: sixty-seven documents were included in the review. At the outcome of their analysis, three roles could have been outlined for the MD thesis: a required academic work (for 41% of the occurrences), a research work (for 40,5%), and a rite of passage (for 18,5%). Six stakeholders were also identified: the medical interns, thesis supervisors, physicians in general practice, General Medicine Departments, thesis commissions and the National College of General Medicine Academics. These stakeholders all considered MD thesis as a collaborative science work that should be enhanced, even if they had sometimes criticised its scientific utility. General Medicine Departments, thesis commissions and National College first regarded the MD thesis as a research work enabling the legitimacy of the discipline; interns and doctors mostly saw it as an academic obligation, with only a symbolic significance, to overcome in order to start their professional career. Conclusion: the MD thesis is assigned to different roles by its stakeholders; the significance of these roles varies according to each stakeholders’ rational expectations.
