Effet des différents supports d’oxygénation non invasifs sur le volume courant : une étude sur banc

Abstract : Introduction: multiple devices are available for non-invasive oxygenation support: oxygen mask, high flow nasal canula (HFNC), continuous positive airway pressure (CPAP), mask non-invasive ventilation (M-NIV) and helmet NIV (H-NIV). There are conflicted data about their respective effects during acute respiratory failure. Tidal volume is a determinant of both efficacy and safety during ventilatory support. We assessed whether the tidal volume (VT) was influenced by non-invasive oxygenation support device. Methods: this six devices were tested using the RespiSim® manikin connected to an ASL 5000® breathing simulator. Three different respiratory mechanics (normal, obstructive and restrictive) and three levels of respiratory effort (low, moderate and respiratory distress) were simulated. Results: as compared to the oxygen mask (VT=440 ± 352 mL), VT increased significantly with M-NIV (VT=690 ± 321 mL) and H-NIV (VT=652 ± 366 mL) and decreased significantly with CPAP (VT=377 ± 297 mL) and HFNC (VT=414 ± 333 mL) (p<0.001). VT was influenced by proximal airway pressure variations (measured into the mouth) generated by the device. Conclusion: in our bench model, VT was significantly influenced by non-invasive oxygenation support devices, whatever the respiratory mechanic or effort simulated. The clinical impact of this mechanical effect needs to be assessed.
