État des lieux de la visite à domicile par les médecins généralistes, en France, en 2016

Abstract : General practitioner home visits hold a key role in primary care. They are an integral part of that profession due to their importance to the quality of care. However, over the last 20 years, there has been a continuous decline in interest in this practice, manifesting itself in a decrease in home visits as a proportion of all general practitioners’ acts. If, in 1980, visits accounted for 38% of the acts, that share had dropped to 12% by 2010. The aim of this thesis is to analyse the number of visits, the actors who practice them and those who benefit from them in France in 2016. This is a quantitative study based on data was provided by the Assurance Maladie et Offre de Soins (the French Public Health Insurance) in response to a questionnaire. The results confirm a decline in general practitioners’ home visits. However, they also show that the majority of doctors still practice home visits, albeit with different frequencies. Finally, the vast majority of patients who receive visits are elderly, and within this population, women with an ALD (Affection de Longue Durée – long-term illness) specifically. Today, demographic and health policy changes, including the development of ambulatory care, should drive the stabilisation of the share of home visits in general medicine acts. However, their inadequate remuneration and the workload of general practitioners contribute to their decline. We must think of ways to reinforce the measures that prove essential in keeping home visits available for patients that need them such as adapted reimbursement, delegation of tasks or adequate medical student education.
