Entretiens thérapeutiques fondés sur le modèle Internal Family Systems : une étude qualitative

Abstract : The Internal Family Systems (IFS) model is a recent systemic individual therapeutic approach. Quantitative preliminary studies suggest its efficacy in anxiety and depressive disorders. However, indications and therapeutic format are not codified, and no qualitative study of IFS has been published in the psychiatric field to this day. The present study explores outpatient psychiatric patients’ experience of an IFS-based psychotherapeutic follow-up, using interpretative phenomenological analysis. Semistructured interviews were conducted with five participants. Five superordinate themes emerged: (1) an deep and visual internal dialogue; (2) a brief and intense work of self-awareness; (3) personality split and reconciliation; (4) the therapeutic relationship between trust and need for autonomy; (5) an emotional and relational assuagement. Therapeutic interviews were seen as surprising, sometime disturbing, and working with psychic multiplicity led participants to question their identity. The therapeutic follow-up allowed the re-experiencing of affects that were split and repressed following traumatic events; the letting go of experiential avoidance; and the reconciling with disowned parts of the self. The therapeutic process of the IFS approach is discussed, bringing it close to psychodynamic therapy. This approach uses regression and personality split as working tools, to conduct a kind of internal psychodrama. The reflexive process of the researcher is also discussed.
