Quelles sont les motivations des chirurgiens bariatriques pouvant expliquer l’explosion de la chirurgie bariatrique en France ? Discussion autour des problématiques sous-jacentes

Abstract : Scientific progress and technological advances have given to medicine the right to treat obesity through bariatric surgery, giving to obesity a surgical value. Bariatric surgery entails a cumbersome process including: preoperative assessment, preoperative multi-disciplinary evaluation, surgical indication and surgical procedure decision-making, and postoperative follow-up. Nonetheless, this onerous path has not controlled the expansion of this type of surgery in France. Our main hypothesis is that tackling the issue of obesity it is not the main motivation to pursue the bariatric surgery specialty. To evaluate this topic, between January and April 2019, we used two approaches: a qualitative method based on semi-structured interviews to French bariatric surgeons in public and private health services; where analysis was performed using the grounded theory and a quantitative method based on a survey to SOFFCO.MM. (French bariatric society); where analysis was performed using Excel and RStudio programs for descriptive and statistical analysis respectively. Twelve surgeons were interviewed and 78 participated in the survey. Motivations about the meaning of surgery, role diversification and valorization, centralization in the healthcare circuit, financial issues and healthcare system adaptation were found. These motivations bring to light the concepts of justice, freedom of choice, normativity, autonomy and paternalism that make us think about what type of bariatric surgery is facing the “obesity challenge”.
