Réduction des risques en tabacologie : identification des leviers et des freins de l’implication des fumeurs dans un challenge antitabac inter-lycées ; thèse qualitative en « focus group » sur les élèves de terminale de quatre lycées bordelais

Abstract : Introduction: the pilot edition of the Challenge Inter Class 2017 puts in competition the senior classes of four high schools in Bordeaux. The aim is to encourage smokers to stop or reduce their smoking during the month of November. This collective challenge allows the class that achieved the best reduction in consumption to win a one-day trip to Futuroscope. With the project to renew the experience in 2018, this study allows us to explore the levers and barriers to participation and success in this challenge. Material and method: this is a multi-centre qualitative study in group interviews or “focus group” carried out with senior students of Bordeaux high schools Albert Le Grand, Brémontier, Magendie and Saint-Genès. All smokers or "challengers" of the classes enrolled in the CIC were invited to participate. The interview guide consisted of ten open-ended, neutral, simple and understandable questions. For each focus group, two organizers were present: the moderator who facilitated the interview, and the observer who took note of the main ideas and non-verbal elements. The data were collected by audio recording and note-taking, and were transcribed verbatim. The analysis was then done by double reading, and grouped the ideas discussed in order to integrate them into a thematic tree. Results: interviews were conducted from December 15, 2017 to January 11, 2018. We conducted 7 focus groups and an individual interview bringing together a total of 33 "challengers". The levers of participation in the Challenge mentioned by the students are the group effect, the concept of challenge, the attraction of reward, the desire to reduce smoking and the questioning of their relationship with addiction. The reduction of smoking in the course of the experiment is encouraged by group spirit, competition, reward, support of the entourage, appreciation of efforts, personal motivation, unannounced control of CO levels and the presence of posters. We have not collected any barriers to participation in the Challenge. The barriers faced by smokers are lack of motivation and smoking by those around them. Conclusion: the notion of a collective challenge coupled with an incentive in the form of a reward seems to be a promising avenue to encourage young people to reduce or stop smoking. The next edition of the Challenge Inter Class will therefore keep the same format and will take into account the criticisms and suggestions of the students interviewed in order to adjust as best as possible to their needs.
