Perception des médicaments génériques : enquête qualitative menée auprès de patients en refus de substitution

Abstract : Generic drugs first appeared 24 years ago in the French Code de la Santé Publique (French Health Code) ; their economic interest is now well recognized. However, they are still a matter of controversy and they have struggled to match their objective of use despite several campaigns and laws in their favour. This study aims at analysing the perception of generic drugs by the French population and more specifically by patients refusing substitution. This study has been performed as a qualitative research consisting of semi-structured interviews conducted with 19 patients who demanded that the note “not to be substituted” appear on their prescription. Its main output is a strong concern from patients, especially regarding the discrepancies of composition with patented drugs. This worry is increased by biased sources of information such as Internet, relatives or media. The physician himself may be a source of this concern because of his own lack of support for generic drugs. As a result, generic drugs are used inconsistently, depending on the pathology and the way the prescription is written. Moreover, regulations ruling the prescription of generic drugs are perceived as increasingly coercive, depriving the patient of his free will. Finally, another impediment to the use of non-proprietary drugs lies in the generic drug itself, because of the variety of its shapes, colours, packaging compared to a unique brand drug. This study reaffirms the central part of the prescriber in the patient’s compliance and the need for a participative medicine approach.
