Infections respiratoires hivernales : pourquoi nos patients consultent

Abstract : Introduction: winter respiratory infections are frequent, often mild and develop spontaneously in the absence of treatment. The aim of this study is to explore patients’ motivations and expectations when they consult a general practitioner for these respiratory infections in order to reduce consultations’ number for this reason. Methods: a qualitative study was conducted by semi-structured interviews, with reasoned recruitment and "snowball effect" in search of maximum variation with double coding and analysis in anchored theory. Results: the consultation is seen as the response to a felt need, motivated by embarrassment, fear of the disease and its evolution or administrative constraint. The feeling of necessity of a medical opinion, a prescription or a certificate is influenced by personal or entourage, society or physician experiences. These influences may not be perceived. Conclusion: perceptions are influenced by a situation of “dependence” on medicine and an over-medicalization of society, mediated by health professionals and media. Physicians often follow a logic of "immediate repair" which generates an over prescription. The media contribute to maintain the perception of a medicine that cures all ills and a vision of well-being to be achieved. It is used by the pharmaceutical industry. Reduce the society medicalization and systematic prescriptions could reduce the general practitioners consultations for winter respiratory infections. Patient empowerment involves to better understand their expectations in order to deliver an information more appropriate and effective.
