Évaluation de la régulation médicale des intoxications accidentelles de l'enfant de 0 à 15 ans au SAMU 40 : étude rétrospective du 01/01/2018 au 31/12/2018

Abstract : Introduction: poisoning is the second leading cause of stroke in pediatrics. There is no protocol to help managing pediatric poisoning at the emergency service coodination centre (ESCC). The purpose of this work is to assess the managing of calls concerning accidental pediatric poisoning, received by the ESCC. Method: this retrospective descriptive mono-centric study focused on calls received concerning accidental poisoning in children 0 to 15 years old at the ESCC at the department Landes, France from January 1, 2018 to December 31, 2018. Results: we have drawn up the classic call of 314 cases. The intoxication concerned a boy (52.7%) aged less than 4 years old (75.2%). He accidentally got intoxicated (78.3%) at home (89%), on a weekend evening (31.2%) between 6 p.m. and 9 p.m. (43.3%), in summer (28.7%). It were the parents (94.4%) who contacted ESCC. The call was mainly managed by the urgent medical assistant (AMU) (70.1%). The toxic offender was a drug (50.3%) including three classes mainly involved (anti-inflammatory drugs (21.5%), paracetamol (17.1%) and psychotropic drugs (10.1%)). Regarding the emergency call, il the patient file, the time of intoxication was not mentioned (73.2%), the amount ingested was well described (98.4%), the child's weight was not given (68.5%) and the clinical evaluation was not described (53, 2%). The poison control center was contacted in 47.1% of cases. The children were not referred for a medical consultation in 79.6% of the cases. When a transfer to the emergency room was decided (15.3%), it was carried out by the parents (74%). Conclusion: the epidemiological data collected in our study seem comparable to that found in literature. However, studies on this subject are few. The managment of pediatric poisoning in the emergency call room is complex. A medical regulation or algorythme protocol within the ESCC may be proposed in order to standardize the management of poisoning in children.
