Prise en charge de l’ostéoporose après fracture sévère par le médecin généraliste chez la patiente âgée : une étude descriptive

Abstract : Objective: osteoporotic fracture is associated with increase morbidity and mortality and is a major problem of public health. Following the GRIO recommendations in 2012, authorizing the prescription of anti-osteoporotic treatment without Bone mineral density after a major fracture, our objective was to evaluate the management of the patient who sustained a major fracture by the general practitioner (GP) one year after. Method: over a six-month period, we included all female patients aged of 75 years or more, caring in Pau’s hospital for a major osteoporotic fracture, untreated with anti-osteoporotic treatment. One year later, we collected data by letters sent to GP and by the reports of hospitalisation. Our primary endpoint was the prescription of specific anti-osteoporotic treatment in one years and / or the specialist consultation. Results: 129 patients were included, we had a response for 47 % of them. One year later, 31 % received an anti-osteoporotic treatment or were consulted a specialist. 83 % received vitamin D therapy. The prescription of anti-osteoporotic treatment was significantly associated with female GP. At the discharge, the prescription or the proposal for prescription concerned 30% of them. Conclusion: despite the recommendations, osteoporosis with fracture remains insufficiently treated as much in the hospital as in ambulatory medicine.
