Analyse des appels des médecins généralistes au SAMU centre-15 : une étude rétrospective au Centre de Réception et de Régulation des Appels de Mont-de-Marsan (40) du 1er janvier 2018 au 31 décembre 2018

Abstract : Introduction: unscheduled urgent acts occupy an important place in the activity of general practicioners in France. This demand comes up against an insufficient medical demography with a density of 160 general practicioners per 100 000 inhabitants in 2017in the Landes, the second largest metropolitan department. The medical and logistical difficulties encountered can motivate a call to the observational study carried out at the SAMU. Material and method: descriptive restrospective single-center observational study carried out at the CRRA in Mont-de-Marsan from 1/01/18 to 12/31/18. All calls from Landes general practicioners to SAMU 40 from their office, home or nursing home were included during this period.The main objective was to analyze general practicioner calls to the SAMU 40 in 2018.The secondary objectives were to assess the consistency between the requests and the decisions and to detail the decisions to commit resources and advice. Results: 2444 files were analyzed, 15,6% (n=382) of requests from general practicioners led to advice, 82,4% (n=2014) involved a commitment of means. 72,3%(n= 1768)were medical transports. The decision to commit ressources seemed independent of the moment of the day. There was an agreement of 89% (k=0,66) between therequests of general practicioners and the decisions of the SAMU. Conclusion: the first reason for calling general practicioners to the SAMU in 2018 concerns the medical transport of patients. The decrease in the participation of private ambulances in favor of non-medicalized taxis under contract aggravates an already precarious situation in the Landes. The quality of the general practicioner/ SAMU interaction may point to the proper funcitoning of the emergency restructuring pact wich, by developing the ambulatory environment, will reduce hospital care and therefore transport requests.
