Consultations pédopsychiatriques aux urgences pédiatriques du CHU de Bordeaux : profil social et sociodémographique sur l’année 2016

Abstract : Introduction: the number of pediatric emergency consultations for psychological or psychiatric reasons continues to rise in France, as well as requests for child psychiatry care. Child psychiatry and social problems are closely related, but few data exist on child psychiatry population in pediatric emergency departments. Our aim was therefore to estimate the percentage of children with social problems among those consulting for psychiatric reasons in pediatric emergency departments. Methods: a monocentric prospective observational study was conducted. All the children who had consulted the paediatric emergency department of Bordeaux University Hospital from 1 January to 31 December 2016 for a psychiatric reason were incluted and those who required the intervention of the child psychiatry team. The main objective was to describe the presence of any social intervention in 2016 or before, including administrative or judicial measures. Results: 33.25% of the 414 children included had contact with social teams before their first visit in 2016. 65% were judicial mesures. 57% were female and the average age was 13 years old. The analysis of children who already had contact with social teams compared to those who never had, found significantly fewer girls (p=0.002), fewer children living with both parents (p<0.0001), more children who had been out of school for more than one month (p=0.0178), and more visits to the emergency department since birth (p=0.0004). On a psychiatric level, children who already had contact with social teams had more known psychiatric history, more psychiatric treatment and more pathologies of the type of behavioural disorders (p<0.0001). Discussion: our results suggest that among all the children consulting in pediatric emergency departments for psychiatric reasons, a significant number are presenting social difficulties.
