Diabètes et grossesses dans l’Ouest Guyanais. Une étude rétrospective à propos de 197 grossesses suivies au Centre Hospitalier de l’Ouest Guyanais de 2013 à 2015

Abstract : INTRODUCTION: We want to update the knowledge of diabetic pregnancies in French Guyana, by describing their characteristics and complications, then by identifying areas for improvement of current care. METHODS: Unicentric retrospective epidemiological survey studying diabetic pregnancies from 01/01/2013 to 31/12/2015. RESULTS: 197 single pregnancies were analyzed. The mothers were 32 years old, with a BMI of 33.13kg/m2, a parity of 4.43 children. The past medical history included: 13.81% high blood pressure, 43.65% miscarriages, 35.52% caesarean sections, 28.29% premature labor, 18.89% macrosomias, 13.41% pre-eclampsia, 9.50% fetal deaths in utero. 70% of the gestational diabetes mellitus (GDM) were diagnosed after 24 weeks of pregnancy, less than 10% were treated by insulin. One third of the women with pre-existing diabetes were not treated before pregnancy. 75% were treated by insulin at the delivery. The complications were: 53.80% anemia, 19.10% premature labor, 13.61% urinary tract infections, 10.34% pre-eclampsia, 7.89% gestational hypertension, 42.37% caesarean section. The children were born at 37.82 weeks of gestation and weighed 6.61lb. 14.84% were premature, 13.87% macrosomes, 9.20% had hypoglycemia. 23.12% required resuscitation, 18.50% hospitalization. CONCLUSION: The prevalence of 2.79% of diabetic pregnancies in French Guyana will increase soon. It is imperative to know the local specificities. The care will be improved by better communication between hospital and other doctors, screening of the GDM from the first pre natal consultation, day hospital for all patients, and a postpartum systematic follow-up.
