La pression intra-orale comme mesure de l’efficacité de la réinnervation laryngée non sélective sur la fuite glottique dans le cadre de la paralysie laryngée unilatérale

Abstract : Objective: the aim of the study is to determine whether the reduction of the subglottic pressure (estimated by the measure of the intra-oral pressure) after a nonselective unilateral laryngeal reinnervation may be used as good indicator to demonstrate the efficacy of this technique on glottic air leak for patients with unilateral vocal fold paralysis.
Method: a retrospective study with data from 50 patients was undertaken. Analyses of different cohorts are done to determine the existence of a significant reduction of the intra-oral pressure during the different times of evaluation (presurgical and postoperative at 3, 6 and 12 months) and whether there are any correlations between these measures and other vocal parameters and subjective questionnaires.
Results: subglottic pressure was found to decrease after the surgery, but not significantly. Moreover, no correlations with other vocal indicators and questionnaires were found.
Conclusion: some bias may influence the results, but the intra-oral pressure does not seem to be a reliable indicator for the reduction of glottic air leak. Other indicators such as oral airflow may be more relevant to evaluate the efficacy of treatment on glottic air leak for patients with unilateral vocal fold paralysis.
