Évolution du parcours de soins des personnes sans-domicile prises en charge par des structures de type "Logement d'Abord"

Abstract : Background: Despite a high prevalence of chronic and complicated diseases, the homeless population’s health care pathway is chaotic with several consults in the emergency department and a very poor follow-up. While providing a home and a personalized support, the « Housing First » program induces an improvement in this health care pathway for homeless persons with psychiatric issues. Objective: Evaluate the « Housing First » program’s effects on the health care pathway for a wider homeless population. Method: Inclusions were made through two associations, Totem and Toi(t) d’Abord, concerning people who had stayed more than 24 months in their apartment. The health care pathway has been evaluated during 12 months before and 24 months after entering the flat, with individual interviews, hospital’s and association’s files. Results: Only 12 persons have agreed to participate, which was not enough to conclude. But some tendencies seemed to appear, as the increase of general practitioners’ declarations and improvement of health cover and social life. Conclusion: This protocol has shown some difficulties, linked to a small number of « Housing First » associations, data protection current regulations, specificities of homeless population and memory bias, and has therefore not been conclusive. However, the tendencies found here are positives and similar to others studies, which encourage to keep up with this model and to pursue further evaluations.
