Analyse des marqueurs acoustiques et vocaux identifiables précocement dans le diagnostic différentiel précoces des aphasies progressives primaires

Abstract : Primary progressive aphasia are pathologies vith various semiologies. They are part of a rigorous diagnostic process requiring a fine clinical analysis to which is added medical imagery. One of the major public health issue nowadays is early diagnosis. However, despite clinical and techonological advances, this diagnosis remains late. With the aim of reducing the diagnostic pathways for some pathologies, research is assessing the value of ecological markers such as the voice. In this contexte, this study is being interested in the contribution of acoustic and vocal analysis, through acoustic markers like jitter or shimmer, but also with linguistic markers like those of speech pause analysis. To do this, we first compared the results obtained in the tests bewteen PPA subjects and controls, then between the differents subtypes of PPA and finally between the PPA group composed of Alzheimer’s disease and fronto-temporal dementia people. The results highlighted the differences between the control group and the PPA subjects, in particular with the number and duration of pauses in an autobiographical story of by lowering the shimmer for patients. The comparison of PPa subjects between them showed, among other things, pauses in agreement with the specific PPA subtype as well as a generally more weathered speech. Finally, the simile with the group made up of MA and DFT subjects revealed more significant breaks for these subjects but, on the other hand, overall more heterogenous performances in our PPA patients. Further acoustic analysis of the different subtypes would be an interesting element to consider in futures studies and in a speech therapy analysis through key tests such as maximum phonation time or speech.
