Tumeur de novo du greffon rénal : une entité particulière ? : étude rétrospective à partir d’une base de données nationale

Abstract : De novo renal allograft tumors are a rare entity. The prevalence is expected to increase due to the aging of the population and the increased lifespan of kidney transplant patient (KTR). There are no recommendations for the management of renal cell carcinoma (RCC) of the allograft. We carried out a multicentric retrospective national study. The data were collected in 32 French transplantation centers between 1988 and 2018. One hundred and twenty-three RCC of the allograft were diagnosed among 56 806 KTR (incidence 0.21%). Mean age at diagnostic was 50.1 years with a 2.24 men/women sex ratio. Mean size was 27.7mm. Eighty-two (78.8%), 108 (94.2%) and 57 (46.3%) were respectively low grade (Fuhrman 1-2), T1 stage and papillary carcinoma subtype. Ninety-six (78%) tumors were diagnosed by routine surveillance imaging. Mean time lapse between transplantation and RCC diagnosis was 143 months. Patients were treated by transplantectomy, partial nephrectomy and focal therapy in respectively 42 (34.4%), 58 (40,6%) and 21 (17.2%) cases. The estimated 5-year cancer specific survival rate was 97%. Mean following time was 60.5 months. Most of RCC allografts are diagnosed incidentally. They were small, low grade with more papilla subtype than in general population. Conservative treatment is feasible and safe, it should be preferred when possible to avoid return to dialysis. Annual screening by imaging seems essential.
