Gingival graft before orthodontic treatment: a clinical case
La greffe gingivale avant traitement d'orthodontie : à propos d'un cas clinique

Nowadays, the combination of orthodontic treatment and periodontal treatment has become essential. These two disciplines have separate objectives, which are specific to each. However, their joint action contributes to obtaining maximum stability of orthodontic treatments in a healthy periodontal environment and allows a reduction in the periodontal risks linked to orthodontic treatments. In the context of this work, the question of gingival grafting before orthodontic treatment is developed through the presentation and argumentation of a complex and interdisciplinary clinical situation, identifying the main decisions made and therapeutic factors. In this clinical case, we have highlighted four factors that are to be taken into account in order to carry out orthodontic treatment in an adequate periodontal context : the periodontal biotype of the patient, the severity of the gingival recession, the feasibility overall treatment of orthodontia and the direction of the orthodontic movement expected.
De nos jours, l’association du traitement d’orthodontie et du traitement parodontal est devenue indispensable. Les deux disciplines présentent des objectifs précis qui leur sont propres, mais leur action conjointe doit contribuer à l’obtention d’une stabilité maximale du traitement orthodontique dans un environnement parodontal sain et permettre la diminution des risques parodontaux liés aux traitements orthodontiques. Dans le cadre de ce travail, la question de la greffe gingivale en amont du traitement d’orthodontie est développée au travers de la présentation et de l’argumentation d’une situation clinique complexe et interdisciplinaire, dans le but d’en dégager les principaux facteurs décisionnels et thérapeutiques. Ainsi, au travers d’un cas clinique, nous avons mis en évidence quatre facteurs à prendre en compte afin de réaliser un traitement d’orthodontie dans un contexte parodontal adéquat : le biotype parodontal du patient, la sévérité de la récession gingivale, la faisabilité globale du traitement d’orthodontie et le sens du mouvement orthodontique prévu.
