L'ostéotomie tibiale de valgisation dans le traitement de la gonarthrose fémoro-tibiale médiale : étude d’une série de 117 cas

Abstract : High tibial osteotomy (HTO) shows good results in the treatment of medial knee osteoarthritis. The aim of this conservative surgery, is to defer total knee arthroplasty (TKA). Aims of this study were the analysis of the survival and factors influencing survival of HTO and clinical outcomes and radiological results. 117 HTO have been included in this retrospective and monocentric study. Mean age was 54,4 ± 7,0 years. 54,7% were men and 45,3% were women. Mean body mass index (BMI) was 29,9 ± 5,6 kg/m2. Joint space narrowing was incomplete for 58,5% (stage 1 and 2 of Ahlback’s classification) and complete loss of joint space was found for 41,5% (stage 3). The five years survival rate was 88,7% and the ten years survival rate was 70,2%. Obesity, a complete loss of joint space (Ahlback stage ≥ 3) and an HKA angle lower than 180° were risk factors for revision to a TKA. At last follow-up, functional results were satisfactory and patients were still undertaking physical activities but favored low-impact sports. Among the 27,4% of complications, lateral hinge fracture was the most frequent. Nonunion was less frequent but leaded to a statistically shorter time to revision to a TKA. With a 10 years survival rate of 70%, this study showed that it is reasonable to perform HTO on young, active and non-obese patients with an incomplete loss of joint space. Undercorrection (HKA angle less than 180°), a major modification of the proximal part of the tibia and a nonunion are to avoid.
