Étude de l’impact thérapeutique des électroencéphalogrammes pathologiques dans une population de patients âgés de plus de 75 ans au sein du CHU de Xavier Arnozan

Abstract : Study of the therapeutic impact before pathological electroencephalogram in a population aged over 75 years in the CHU of Xavier Arnozan Objective: determine the percentage of therapeutic impact before a pathological electroencephalogram (EEG) Methods: all patients aged 75 or over who having an EEG at Xavier Arnozan University Hospital from 08/10/2017 to 11/30/2019 were enrolled. The study was retrospective in a unique center, observational requiring the collection of clinical data. The main endpoint was the percentage of therapeutic modifications of psychotropic drugs (antiepileptics, benzodiazepines, antidepressants, neuroleptics) because of a pathological EEG (paroxysms, slow focal focus, encephalopathy / slowing down). Results: among the 150 patients (mean age: 85 years, 57% women) with a pathological EEG, 115 (76%) patients benefited from a therapeutic modification. Among these indications, focal abnormalities (paroxysm and slow wave) were most often encountered in the case of clones and focal neurological deficits (25 and 27%). Traces of encephalopathy were more frequent in the case of altered level of consciousness (28%). Hallucinations and falls were associated with a high rate of normal EEG (80 and 75%, respectively). Conclusion: the EEG is a simple, non-invasive examination, which, despite its lack of specificity and sensitivity, has a strong therapeutic impact when it is pathological.
