Impact du clampage retardé du cordon ombilical sur l'ictère néonatal chez le nouveau-né à terme à la maternité de Chambéry

Abstract : Objectives: The delayed umbilical cord clamping allows placental transfusion, and improve blood volume at birth. Ones of many clinical benefits are a physiological transition, a decrease in early childhood anemia, and a better neuro-development. The higher neonatal hemoglobin level is a theoretical risk of jaundice. The aim of this study is to investigate the impact of delayed umbilical cord clamping on neonatal jaundice in terms neonates. Method: This is a retrospective cohort study before and after implementation of a delayed cord clamping protocol at maternity of Chambéry, France. Low risk term neonates were included in cesarean section or vaginal delivery. The primary outcome measure was the transcutaneous bilirubin level. The secondaries outcomes are number of serum bilirubin levels drawn, the necessity to treat by intensive phototherapy, apgar score, cord blood ph and maternal haemorrhage at birth. Results: 350 mother-child couples are included in this study. Transcutaneous bilirubine levels increased at day 1 and 2 in delayed cord clamping group. Number of serum bilirubin levels drawn increased also. Apgar score < 7 at 1 minute was lower. There was no difference between the necessity to treat by intensive phototherapy/ phototherpy rates, gaz analys and mathernal haemorrhage at birth. Conclusion: Delayed cord clamping for low risk term neonates was associated with higher bilirubin level the first two days, but without increasing risk of neonatal jaundice requiring phototherapy.
