Influence de l’inflammation associée à un épisode infectieux aigu sur la métabolisation des médicaments substrats des cytochromes P450

Abstract : In order to personalize drug management, it is important to identify the factors of pharmacokinetic variability, especially for drugs with a narrow therapeutic range for which pharmacological therapeutic monitoring is recommended. Inflammation, a protective response against acute infections and injuries, contributes to intra- and inter-individual variability in drug exposure by modulating the activity of enzymes and transporters involved in drug metabolism, and by modifying their binding to plasma proteins. To better understand the variability of drug exposure, it is important to know the impact of inflammation on drug metabolism and the clinical consequences. This work first presents the molecular mechanisms by which inflammation can inhibit drug-metabolizing enzymes and then reports the consequences of the occurrence of infectious inflammatory events on exposure to drugs with narrow therapeutic ranges and substrates of cytochromes P450, as well as the clinical consequences. Finally, in the context of the SARS-CoV-2 (COVID-19) pandemic, a pathology associated with severe acute inflammation, we reported examples of pharmacokinetic changes of narrow therapeutic range drugs observed in COVID-19 patients.
