Le lien ville hôpital pour le suivi pédiatrique dans le bassin grassois : attentes des médecins

Abstract : Introduction : Parents are free to choose their children health follow up. This pediatric care can be performed by a private pediatrician, a general practitionner, an hospital pediatrician or by the so called « PMI » : the local Mother and Child Protection organization. At the present time, there is a lack of private pediatricians in the area. As a consequence, the strenghtening of the town-hospital-network is essential to ensure a reliable pediatric monitoring on the Grasse territory.
Aim : Learning about doctors’ expectations in order to strenghten the town hospital network.
Method : Qualitative study using individual semi-structured intrviews of, pediatrician practicing in the hospital, private pediatrician and general practitionners in or around Grasse.
Results : This study shows difficulties on both collaboration and communication. There is a lack of interaction between private pediatricians and hospital pediatricians. Whereas, the strenghtening of their collaboration is a real deal. Other concerns regarding accessibility (switchboard service, unknown pediatrician on duty phone number, delays regarding medical report reception…) and communication channels (not synchronized and unsecured emails infrastructure). Information or learning meetings (last recommandations, care consensus) can be implemented in order to improve team building, for volunteers. A structured organization based on an official network is not a goal yet for doctors interviewed in this study.
Conclusion : The town-hospital network has to be strenghtened among pediatricians and general practitionners. Both formal and non-formal meetings must be impletemented accordingly to doctors expectations and existing trainings. Regarding accessibility, information, feedbacks and best pratices must be strongly known and shared (for instance set up an on duty pediatrician phone number available for all doctors in the area). Besides, further initiatives have to be enhanced : single e-mail address for pediatrician on duty, private and secure messaging among pediatricians and general practionners, intranet/internet websites… Besides, a private and mapped directory database including phone numbers and medical office addresses will ease the communication. The implementation of the so called « Communauté Professionnelle Territoriale de Santé » (CPTS) will definitively foster these initiatives on Grasse territory.
