Efficacy and safety of a standardized hyperbaric oxygen therapy protocol for retinal artery occlusion

Abstract : Purpose: To assess the efficacy and safety of a standardized Hyperbaric Oxygen Treatment (HBOT) protocol in patients undergoing a Retinal Artery Occlusion (RAO).
Design: A retrospective study was conducted in our tertiary care centre from July 2016 to September 2019.
Participants: Patients undergoing Central Retinal Artery Occlusion (CRAO) and Branch Retinal Artery Occlusion (BRAO), for less than seven days.
Methods: Once the diagnosis was established, the patient was urgently referred to the HBOT department for the first hyperbaric chamber session for 90 minutes at a pressure of 2.5 ATA. The patient benefited from two daily sessions seven days a week, for at least 15 days. If no perfusion was noted on fluorescein angiogram performed at Day 15, the treatment was continued for an additional week with a check-up at Day 21. Main outcome measures: BCVA improvement was defined as a decrease of 0.3 logMAR at one month.
Results: Twenty-eight patients were included during the study period. 57% of patients were treated more than 12 hours after the onset of the first symptoms. Mean BCVA was 1.5 logMAR at referral and improved to 0.9 logMAR following HBOT (p=0.001). Multivariate analysis identified high blood pressure (p=0.039) and low initial BCVA (p=0.005) as poor prognosis factors.
Conclusion: Performing HBOT sessions twice daily at 2.5 ATA pressure appears to be an effective and safe treatment for RAO. Half of our patients underwent BCVA improvement. The implementation of an international and standardized HBOT protocol is warranted for further prospective and comparative studies.
