Quels infirmiers pour les patients en situations palliatives avancées et terminales à domicile ? Enquête réalisée auprès des médecins généralistes de Gironde en 2019

Abstract : Objectives: to describe in which extent the different types of nurses are involved in taking care of advanced and terminal home palliative care patients. Material & methods: survey of practice by online questionnaire carried out among general practitioners in the Gironde region (France). Palliative situations were identified by the use of the SPICT-FR tool. Results: 89 GP answered the form and described 607 patients who are in advanced and terminal palliative situation, and 260 patients who died at home after palliative stage diseases during the last 12 months. The local practice nurses, alone, take care of 64% of the ongoing situations, and 56% of the patients the month before their death. Local practice nurses are also involved in the majority of the situation where « Home health agencies » and « Home nursing services » take action. Conclusion: while common representation tends to consider « Home health agencies » as major actor of the advanced and terminal home palliative care situations, this survey shows local practice nurses have a preponderant part in these situations. Home palliative care policies needs to support these actors to widespread palliative culture.
