Surpoids et obésité de l’enfant : freins à la prise en charge par les médecins généralistes

Abstract : Context: Childhood obesity is a global public health problem with physical and psychological consequences for children, but also for society as a whole. Objective: This study aimed to explore the barriers to the management of overweight and childhood obesity encountered by general practitioners (GPs) in Savoie and Isère who are not members of the RéPPOP. Method: This qualitative study was conducted by two investigators that carried out 14 semi-structured interviews of established GPs. Results: The GPs recognized the importance of their role in the management of this disease, but described several categories of barriers related to the GP, the child and family, pathology and society. They mentioned theoretical and practical shortcomings as well as a lack of training. Lack of time was also raised. Representations of the state of being overweight and obesity has led to an avoidant behaviour on the part of both GPs and families, along with difficulties in the doctor-patient relationship, linked to the psycho-social dimension of this pathology. Finally, GPs mentioned obstacles linked to health policies and the organization of care with a lack of fluidity in the channels, a limited supply of care and a late patient care. GPs considered that childhood obesity was a complex problem in the face of which they could feel powerless. Conclusion: The GPs knowledge building, early prevention, multidisciplinary care and the implementation of specific government actions would make it possible to prevent the developpement of obesity in children, and thus limit the difficulties and failures of its management.
