Caractéristiques des patients diabétiques de type 2 accueillis à la permanence d’accès aux soins de santé de Montpellier : étude prospective sur douze mois

Abstract : Introduction. Type 2 diabetes mellitus (T2DM) particularly affects people in state of social deprivation, yet less studied than the general population, justifying our research question: what are the characteristics of patients with type 2 diabetes who are admitted to the Permanence d’Accès aux Soins de Santé (PASS) of Montpellier? Method. This prospective descriptive cross-sectional study was conducted thanks to a face-to-face anonymized questionnaire with telephone interpreting. The main objective was to describe the patients with a known T2DM cared at the PASS for one year. Results. Twenty-two patients were interviewed: 13 men and 9 women, with a median age of 45.5 years. They were mostly obese or overweight. The median glycated hemoglobin was 7.95%. The complications concerned 36% of the patients. They were socially isolated in 59.1% of cases and 86.4% had no financial resources. Only 13.6% had a fixed address and 68% were not French-speaking. Diabetes was « a serious illness » for 81.8% of respondents and « very much concerned » 72.7% of them. Most had adapted their diet and physical activity following the diagnosis but recognized obstacles to the application of dietary rules. They lacked keys to assess the balance of T2DM. All were interested in an information program about T2DM. Conclusion. The diabetic patients of the PASS of Montpellier combine social vulnerabilities. Despite this, they are concerned about the disease and have a strong demand for information. An appropriate therapeutic education could improve glycemic balance and contribute to strengthen the social bond.
