Paiement à la performance : financement et accès au marché des médicaments innovants en France et dans le monde, étude de cas des CAR-T cellss

Abstract : The concept of performance retains a prominent place in the word of health. Numerous mechanisms are in place in different countries of the word to welcome medicam innovations as quickly as possible. At a time when innovative medicines are arriving faster on the market, their rising prices are becoming a real burden for paying agencies. Many tools for financial regulation of medicines exist for this purpose. But the uncertainty of the effectivness of these innovative drugs implements the budget of the paying agencies which takes a real risk by funding. CAR-T cells Kymriah® and Yescarta® are among these innovative and expensive drugs. The study of their access to the market in France, Germany, the United Kingdom, the United States and Japon shows a great diversity of consideration of prices and reimbursment of innovation in the word. Pay-for-performance contracts based on the risk sharing to finance expensive medicines, including CAR-T cells, could be a relevant and laudable alternative to the cost of medicines in the future. This conclusion is based on the analysis of 10 cost-effectivness studies of Kymriah® and Yescarta®, allowing us to conclude on their profitability and the usefulness of performance-based payment contrats.
