Évolution de la morbi-mortalité des patients atteints de mucoviscidose à la Réunion entre 2006 et 2017

Abstract : Background: Cystic fibrosis is a lethal autosomal recessive disorder with high prevalence and incidence in Reunion island, a French overseas territory. Objectives: To describe morbidity and mortality in cystic fibrosis patients from Reunion island. Methods: A retrospective observational cohort study, using data from the French Cystic Fibrosis Registry from 2006 to 2017. Results: 157 patients were included (87 males, median age at diagnosis: 1 month 0-8). The proportion of adults increased during the study period and accounted for 37% of patients in 2017. Seven parameters were significantly associated with lower Predicted Forced Expiratory Volume in 1 sec (FEV1): duration of follow-up (p<0.001), date of birth (p<0.05), baseline FEV1, F508del/Y122X genotype (p<0.05), chronic colonization with Pseudomonas aeruginosa (p<0.001), positive respiratory culture for Nontuberculous mycobacteria (p<0.05) and underweight (p< 0.001). Cystic fibrosis-related diabetes and Gastro-Esophageal Reflux were identified as emerging chronic comorbidities, with 42% of adults with diabetes (22/53) and an overall reflux prevalence of 49% (67/138) in 2017. Sixteen patients (10%) died during the study and cardiorespiratory failure accounted for 56% of deaths. Mortality decreased from 18 to 2.4/1000 during the period but survival remained lower in Reunion island compared to metropolitan France (p<0,001). Discussion: Mortality decreased during the last decade in patients with cystic fibrosis from Reunion island. However, survival remains lower as compared to metropolitan France. Difference was less significant in younger patients, probably due to a better harmonization of clinical practices.
