Stratégies de prise en charge de la colique néphrétique à SOS Médecins Bordeaux

Abstract : Background: recommendations for renal colic have been made by the French Society of Emergency Medicine, as well as by the European Association of Urology ; but not in primary care. The aim is to analyze and evaluate the professional practices of SOS Médecins Bordeaux practitioners in the management of renal colic. Method: first, we carried out a retrospective analysis of all the patients diagnosed with “renal colic” in 2018, at SOS Médecins Bordeaux. Secondly, we carried out a prospective observational study of patients with renal colic in 2019, thanks to a questionnaire sent to the 96 physicians of the structure, which included the management of a patient, and had to be completed after the consultation. The primary outcome measure was the rate of application of professional practice recommendations. Results: following the retrospective analysis, we included 2,208 patients from January 1 to December 31, 2018. The average age was 44 years, men appeared 1.17 times more affected than women, and 17% of the patients had a background history of urolithiasis. The hospitalization rate for renal colic was 16%. During the prospective study, 124 patients were included, from August 16 to December 31, 2019. We have shown that the rate of application of recommendations for the renal colic, was 77 % for the use of non-steroidal anti-inflammatory drugs, 60% by using urinary strips, 58% for creatinine dosage, and 33% for the prescription of appropriate imaging (plain abdominal ultrasound or abdomino-pelvic scanner). Conclusion: the practitioners of SOS Médecins Bordeaux have applied the recommendations, but the radiological management needs to be improved. Difficulties in accessing emergency additional tests in ambulatory care can be an obstacle to the strict application of these points, requiring better coordination between specialists and general practitioners.
