Collaboration des médecins généralistes avec les structures de soins palliatifs hospitalières de Sud Gironde en 2018 : enquête d’opinion

Abstract : Introduction: death at home is a major expectation of the French people. However, the quality of palliative care at home is unsatisfying, due to challenging coordination and territorial inequity in access to palliative care. Objectives: the primary objective is to assess General Practitioners (GP) satisfaction rate regarding the care for their patients, died in 2018, by hospice palliative care structures of Sud Gironde. The secondary objectives are to highlight the difficulties encountered and suggestions to make improvements. Materials and Methods: a mixed, descriptive, retrospective and monocentric study has been conducted using a self-administered questionnaire by GP between April and June 2019. Results: 45 GP in 77 responded, or 59.7%. Most of the GP care for 1 to 4 palliative patients each year. In total, 60.1% reported having no palliative care training. The satisfaction rates of GP concerning the structures were 94.1% for Home-based Palliative Care Teams (HPCT), 84.8% for Home Care Services (HCS) and 95.0% for Palliative Care Beds (PCB). The difficulties encountered by GP are related to their involvement in care, communication, psychosocial support, availability and reactivity of palliative care structures as well as lack of knowledge on the structures. Suggested areas for improvements were to multiply discussions through multidisciplinary meetings and innovative communication tools, along with an increase of the PCB capacity. Conclusion: the areas to explore for answering the territorial needs are: hotline, telemedicine, development of a tool for collegial procedures, creation of an emergency bed and implementation of training evenings.
