Évaluation à un an de l’efficacité du suivi par protocole de coopération interprofessionnelle des patients sous pompe à insuline et de l’impact sur l’organisation des soins

Abstract : BACKGROUND: Despite clear, evidence-based glycemic targets in diabetes, the current evidence supports difficulties for patients to achieve optimal glycemic control. This leads us to consider new methods of patients support, including the interprofessional cooperation protocol (ICP): "Nurse management of diabetic patients treated with external insulin pumps according to an Electronic Personalized Education Plan (ePEP) with prescriptions and primary care in place of the physician". The objective of our study is to evaluate the effectiveness of this new monitoring method on glycemic control. MATERIALS AND METHOD: Monocentric, retrospective, observational, controlled study including type 1 and 2 diabetic patients treated with insulin pumps. For the group benefiting from ICP, inclusion of any patient included in the cooperation protocol between 01/11/2018 and 31/05/2019. For the group benefiting from conventional follow-up only, patients were selected from the database of the service provider Agir à Dom. RESULTS: 116 patients were included: 40 in cooperation protocol and 76 in conventional follow-up. At 12 months of follow-up, the non-inferiority of the cooperation protocol to the 0.4% threshold was demonstrated on the difference between HbA1c deltas, which is 0.02% (95% CI: [-∞; 0.35] and p= 0.029) but not on the mean difference in HbA1c, which is 0.33% (95% CI: [-∞; 0.66] and p= 0.34).
