Mesure isotopique des volumes sanguins : validation de l’asepsie des préparations requises et habilitation des opérateurs impliqués

Abstract : In nuclear medicine, radioisotopic cell labelling is carried out for some examinations such as isotopic measurement of blood volumes. At the University Hospital of Bordeaux, following the marketing withdrawal of chromium-51, a new method was implemented for this examination: autologous red blood cells radiolabelled with metastable technetium-99 are now used for the measurement of red blood cell volume and human albumin radiolabelled with iodine-125 is used for the measurement of plasma volume. The aseptic preparation processes involved are complex and multi-step. They are performed in an open system and therefore carry a high risk of microbiological contamination. As part of a quality assurance approach and securing patient care, we wanted to ensure the control of our preparations’ microbiological contamination. To do so, we validated the asepsis of our preparation processes through the development of two challenged aseptic simulation tests and a sterility test with a rapid alternative method adapted to the preparation of red blood cells radiolabelled with metastable technetium-99. In addition, in our department, isotopic measurement of blood volumes is delegated to nuclear medicine technologists, under the technical responsibility of radiopharmacists. We therefore created and implemented a qualification program for operators. It includes a theoretical training, the execution of the aseptic simulation tests and the sterility test developed, as well as an audit of practices based on an evaluation grid adapted to the entire circuit of blood volumes’ isotopic measurement.
