Évaluation de l’efficacité d’une campagne de vaccination antitétanique menée chez les patients de plus de 65 ans d’une maison de sante pluriprofessionnelle en Isère

Abstract : Introduction: Tetanus is a serious disease, which is currently rare in France. However, it is entirely avoidable thanks to vaccination, which is totally effective and with few sides effects. That’s why the Izeaux health center wanted to offer its patients aged over 65, population the most affected by tetanus, a tetanus vaccination campaign, in order to update their vaccination status. Materials and methods: this is an observational, retro-prospective, single-center, comparative study, conducted from March 2019 to March 2020. Included patients were 65 years old and over, and their general practitioner worked at the health center. The tetanus vaccination status was determined for each patient at inclusion. A letter was sent to patients who were not up to date, invited them to join the vaccination campaign. 6 months later, patients who were still not up to date were contacted by phone. The final assessment was performed 12 months later. The primary objective was to determine the effectiveness of the vaccination campaign on the evolution of the vaccination rate. The others objectives were to identify determinants of tetanus vaccination, and calculate the cost of this campaign. Results: 584 patients were included : 228 were up to date for tetanus (39,04% [IC95% : 35,08-43,00]). 12 months later, 188 patients had been vaccinated, compared to 48 the previous year. The number of vaccinations increased by 391%. At the end of the campaign, the tetanus vaccination coverage of the target population was 77.74% [95% CI: 74.37-81.11]. The secondary results show that the general practitioner is one of the main determinant of vaccination (p <0.01). Conclusion: this study suggests that a local vaccination campaign is an effective means to improve the tetanus vaccination coverage.
