Obstacles au rattrapage vaccinal chez les migrants primo-arrivants et solutions pour y remédier

Abstract : Background: newly arrived migrant populations in France are exposed to multiple vulnerabilities and have a higher risk of contracting infectious diseases preventable by vaccines. They often have incomplete coverage compared to french health policy recommendations. Most of the time, they don’t have a vaccination record. This raises the question, in front of an unknown immune status, of what catch-up vaccination strategy can be adopted. Primary objective was to analyse catch-up immunisation’s barriers encountered by doctors in Bordeaux with migrants and solutions to overcome these barriers. Methods: an observational qualitative study with 13 semi-structured interviews, led between January and June 2019, was conducted. A double open axial coding was realized with Nvivo® software, according to the grounded theory. Results: practices among interviewed doctors regarding catch-up immunisation strategies are very heterogeneous. Several barriers have been identified, linked to patients, doctors, organizations and health care system. Some solutions have been established to overcome these barriers and to improve health care and vaccination access to migrants. Conclusion: the Haute Autorité de Santé (HAS) recommendations, published at the end of 2019, shed light on some points, especially on the strategy faced with an unknown vaccination status or the use of serology. However, some problems mentioned by interviewed doctors deserve to be addressed such as improving the coordination between the different organizations which are helping migrants and with general practitioners.
