Revue méthodologique des meilleures études d'impact sur la santé

Abstract : Numerous aspects of human life affect health, notably the environment and public policy. In order to quantify this influence at the scale of populations, the health impact assessment approach was developed to translate the measures of associations available into something more meaningful for public policies. Our aim was to review the methodology of health impact assessment studies. To do this, we performed a non-systematic review of the scientific literature with a state-of-the-art approach. Health impact assessment studies typically follow the steps of Screening, Scoping, Assessment and Reporting. The key steps are to establish an exhaustive list of environmental factors influenced by the policy, to choose the exposure-response function associated to each factor-outcome pair and to produce a full uncertainty analysis. Inconsistently, it includes the steps of socio-economic analysis, uncertainty analysis and evaluation. The outcome of the study will either be quantitative or qualitative. At certain steps, notably Screening and Reporting, community and stakeholder participation is possible. Key issues are the availability of the exposure response functions, the necessity to take into account, simultaneously, several factors with inter-relations, and to obtain a useful result despite the possibly strong uncertainties. Furthermore, confusing terminology with numerous acronyms can impede a necessary consensus. Pending issues include proposing a formal approach to the quantitative handling of the level of evidence and other technical issues related to the simultaneous consideration of several exposures in particular when some of these exposures are causally related.
