Perceptions et ressenti des sages-femmes de Marseille face au questionnement éthique soulevé lors de la prise en charge et de l'accompagnement des interruptions médicales de grossesse en maternité

Abstract : Introduction: With more than 7000 termination of pregnangy (TOP) each year in France, medical and psychological care for requesting couples is frequent in maternity hospitals that practice it. Possible at any time during pregnancy, it gives rise to many ethical reflections and pushes midwives to confront the distress of the parents. The research question raised is the following : "What are the perceptions and feelings of midwives in Marseille regarding the ethical questioning raised during the care and support of medical termination of pregnancy in maternity wards ? ». Objectives of the study: To identify and analyse the perceptions and feelings of midwives in Marseille regarding the ethical questioning raised during the care and the support of TOP in maternity wards, and to identify the coping strategies put in place by midwives to facilitate this care and support. Finally, to propose tools likely to improve the position of midwives towards TOP in order to improve care and support for these couples. Method: This is a phenomenological qualitative study conducted by semi-directive interviews with volunteer midwives using a pre-tested question grid. The study includes 10 interviews, ranging in length from 30 to 60 minutes and conducted between July and November 2019. Results : TOP is a common practice that raises many thoughts among midwives, particularly on the role of society in the integration of disability. This procedure, known to be stressful for parents, is also proving to be difficult for some midwives. Professional and personal experience appears to be the main help in managing their emotions and improving the experience of this accompaniment. Conclusion: Rarely in ethical conflict with TOP, midwives put the interests of couples first. However, they are in a delicate position, being both at the end of the chain and on the front line. Supporting TOP is not easy, to the point that some midwives are running away from it. It is therefore important to support the professionals by freeing their word and developing their training, but also by improving the quality of support for parents by giving them other possibilities and by developing the medical care.
