Expérience Bordelaise du traitement percutané par clip de l’insuffisance mitrale fonctionnelle, en contexte de dysfonction ventriculaire gauche

Abstract : Introduction: mitraclip does not have Marketing Authorization for the treatment of functional mitral insufficiency in the context of ventricular dysfunction. The objective of our study is to determine the efficacy and safety of the procedure in the Bordelaise cohort. Methods: we realized a retrospective analysis of patients receiving Mitraclip in the context of functional mitral insufficiency with LVEF ⩽40% as compared to patients treated for organic mitral insufficiency. Clinical and medical datas were collected before, during and immediately after procedure, at 1 month, 6 months and 1 year. The primary outcome included a composite endpoint of all causes mortality and hospitalizations for heart failure. Results: one hundred and four patients were included. Thirty-four in functional mitral regurgitation with LVEF ⩽ 40% and 70 in organic mitral regurgitation. The primary endpoint occured in 53.1% of the functional group. 29.4% died and 44.4% had a recent hospitalization for a non-lethal heart failure episode. There was no improvement in mortality rate, but a significant improvement in clinical and ultrasound parameters. Compared with patients presenting an organic mitral regurgitation, there was significantly more complications, mostly hemodynamic, but the result on mitral insufficiency was comparable at one year among survivors, at an average of 86% low grade mitral regurgitation. Conclusion: the implantation of Mitraclip in context of functional mitral insufficiency associated with left ventricular dysfunction does not seem to improve mortality at one year, but seems to improve quality of life and ultrasound parameters. The procedure has a high rate of complications, but a great result on regurgitation at one year.
