Ressenti des médecins généralistes, infirmières et patients sur le protocole ASALEE dans les Landes : évaluation qualitative croisée par méthode focus group

Abstract : Thanks to the cooperation between general practitioners (GP) and public health nurses, ASALEE has proved its efficiency with chronically ill patients. Several qualitative studies (mainly semi-structured interviews) have been looking into the perception of each members of the trio (GPs, nurses or patients) but none of these has crossed their results. What is the crossed perception of these different actors? Can ASALEE project be improved thanks to these results? We conducted a crossed qualitative evaluation by focus group method. Participants pointed out the availability of nurses, their responsiveness and caring, their comprehensive patient-centred care. This enables a relationship of trust which promotes patient autonomy. The administrative simplicity was appreciated as its accessibility whether financial (full costs covered by the Health Care System) and geographic (therapeutic education in ambulatory care). Team work seems to be boosted by the GPs/nurses cooperation and lead to a change in medical practice. The holacratic functioning brings a flexibility to patients’ needs and capacity to innovate. A lack of communication from the association has been noticed. Misunderstandings between nurses and others health professionals are sometimes reported. The lack of training for GPs could limit their support and involvement. Some organisational and psychologic difficulties have been felt by nurses. Questions about the sustainability of funding by the Health Care System and the possible role of private actors have been raised. The place of advanced practice nurses has also been discussed. The potential improvements were about communication, the accessibility of trainings, the coordination between the different educative programs and extension of the nurse skills.
