Flux et réserves sanguines myocardiques sur gamma camera CZT : corrélations avec l’état coronarien du patient et avec plusieurs facteurs de risques cardio-vasculaires

Abstract : Context: there is a growing interest of the scientific and medical community in regard of quantitative evaluation of myocardial blood flow (MBF) in coronary artery disease (CAD). Several imaging techniques, with scintigraphy among them, have adapted to provide a quantitative MBF measurement. This is a study designed to test association between stressinduced MBF (sMBF), resting MBF (rMBF), MBF reserve (MFR) and CAD in a population of CAD and non-coronary patients. Secondary objectives were to confront visual analysis and dynamic analysis and to explore potential association between MBF and several cardiovascular risk factors Methods: a total of 155 patients who underwent dynamic myocardial perfusion imaging on a CZT camera were included. sMBF, rMBF, and MFR were evaluated, and cardiovascular risk was assessed. Results. Significantly lower sMBF and MFR were observed in CAD patients in global analyses. Logistic regression analysis showed that the risk of CAD is increased by a factor 3.22 for a unit decrease in sMBF (OR=3.22, [1.56;7.14], p=0.002) and the risk of ischemia or necrosis is increased by a factor 10 for a unit decrease in MFR (OR=10 [1.56;100 p=0.038). A significant correlation between total sMBF, rMBF, MFR, and diabetes, age and sex was found. Conclusion: sMBF and MFR as assessed on CZT gamma-cameras can be used to determine the coronary state. Low total sMBF might be an independent risk factor of coronaropathy. An inverse correlation was suggested between total rMBF and sMBF with diabetes and between MFR and age or sex.
