Indicateurs et DataVisualisation de la qualité de prise en charge des Sarcomes : création de l’application VizaSarc

Abstract : Introduction: the management of sarcomas is complex because of their rarity and heterogeneity. Poor diagnostic and surgical practices decrease patient survival. In 2010, a network of expert centers was formed and a national database (Sarcoma BCB) was created. The objective was to develop an application for the automatic calculation of management quality indicators from national data and with data visualization tools. Method: a multidisciplinary group of 7 experts defined 9 indicators. The study population was soft tissue and bone sarcomas diagnosed between 2010 and 2019 in an adult patient. The app was developed with R Shiny. Results: the dashboard displays indicators over a year, for a center or nationally. The visual tools are an interactive flowchart, overall survival curve, bar, donut, and Euler charts. In 2019, indicators M1 (imaging), M2 (size / depth), M3a (biopsy), M5 (re-reading addressed) and O1 (biopsy) are greater than 75%, M3b (microbiopsy) and M4a (R0 inside network) are slightly greater than 50%, M4b (R0 outside network) and M6 (cryopreservation) are 27% and 38%. M1, M3 and O1 indicators have evolved positively since 2010. M4 indicators have started to decrease. Discussion: the application allows a quick visualization of the state of practices and their evolution. Sankey diagram and donut charts help practitioners to explore and better understand the results. Despite encouraging results, moderate and low adequacy rates of some indicators show we need to continu efforts to improve the management of sarcomas.
