Connaissances, représentations et attentes des patients à propos des placebos impurs en médecine générale : approche qualitative

Abstract : Background : Impure placebos are substances or methods which have a known efficacy, but whose evidences of a specific effect are not present in the afflictions for which they are prescribed. They are commonly prescribed by general practicionners. The aim was to study the knowledges, représentations and expectancies of patients about impure placebos. Method : This work was a qualitative study, inspired by the grounded theory, using semi-structured interviews. The interviews were analysed by two independant investigators to guarantee data triangulation. Results : Fourteen interviews were conducted. Impure placebos were imprecisely positionned between pure placebos and medication. The felt efficacy was variable and a matter of personnal interpretation. The « other patients » were regarded as more willing to ask for impure placebos. The patients expected a personnal efficacy and a benefit, using a pragmatic more than scientific reasoning. They trusted their doctor. The doctor effect had an important part in the prescription. The impure placebo prescription could be ethical under specific conditions, when there was a quality doctor-patient relashionship. Conclusion : This study found various and unstable representations of impure placebos. Efficacy was person dependent. The main expectancy was personnal efficacy, leading to a benefit. The open-label placebo administration could allow the use of impure placebos in some clinic situations, to increase the effectiveness of the care, in an ethical manner, in a perspective of an optimized Evidence Based Medicine.
